The Ultimate Feedback Template

Use these questions the next time you ask learners for feedback:

  1. What went well [today OR this week - pick one]? 

  2. What could be improved?

  3. What do you appreciate about our learning community? 

  4. What would you like to see less of in our learning community?

  5. What would you like to see more of in our learning community? 

  6. Anything else [your name - Ex. Ms. Folgers] should know?

    AND NOTHING MORE. Why short? It is immediate to do and immediate to implement for you and your learners.

Your Only Decision: Anonymous or not? 

Whether your feedback survey is digital or on paper, you must decide if you ask for names or keep the survey anonymous. Consider the following: 

  • Anonymous 

    • Positive: Expect HIGHLY actionable and very honest feedback

    • Considerations: Unable to ask follow-up questions or address individual needs

  • Name required

    • Positives: Able to follow up on individuals’ questions, comments, and concerns

    • Considerations: Still honest, but some people see “Name” in education and fear it might impact their ‘grade, and so they adjust their responses accordingly

  • OR have an optional name space with the caption, “Name (optional, so that I can follow up if needed)”

    • If your feedback form is digital and anonymous, make sure your form does not auto-populate people’s email
